We offer assistance to plan and self-managed participants, as part of therapeutic supports. We run groups classes, workshops and one-on-one individual consultations to help participants reach their health and well-being goals.

 

MEDICARE AUSTRALIA

Referrals from GPs for Chronic Disease Management/Enhanced Primary Care (EPC) Plans, Team Care Arrangements, and Diabetes Health Plans are accepted.

Thrive Dietetics is a provider for the Department of Veterans Affairs (DVA) and is able to provide bulk-billed services through the D904 scheme.
